8VAC20-780-500-B · Based on observation of a combined classroom caring for infants and toddlers it was determined that not all procedures for diapering were followed. Evidence: On 01/25/2024 at approximately 1:10 pm the LI (Licensing Inspector) observed a classroom of combined infants and toddlers. The room was combined due to a malfunction with the facilities water and therefore these classrooms were displaced and combined in a larger room known as "The Brooke." The room was divided with infants in one half of the room and toddlers on the other half of the room. The children were being diapered on a changing pad on the floor and not a changing table as required for children two years and younger.

Evidence: A spray bottle of disinfectant was observed on top a low refrigerator and not locked as required. The children being cared for in "The Brooke" ranged in age from infants - toddlers.

Evidence: On August 28, 2023 three staff were caring for more than 12 infants from 9:11 am - 9:45 am. At 9:45 am a fourth staff person arrived bringing the classroom back into the appropriate 1:4 staff-to-child ratio.

8VAC20-780-340-A · Based on review of video footage, interviews and review of documentation, it was determined that staff did not always ensure the care, protection and guidance of all children in care. Evidence: Child #1, age 13 months, was observed on video playing with blocks stored on top of a book case in the infant room. Staff #1 is observed picking up the tablet and viewing the screen. Staff #1 then snatches the blocks from Child #1 and replaces them on the shelf. Child #1 stays at the bookshelf. Staff #1 is then seen taking Child #1 by the left arm and carrying Child #1 by the left arm approximately eight feet to the carpet and sitting Child #1 down.

Evidence: On October 12, 2022 Child A, age 2 years old, got her finger stuck in a toy tambourine. As a result, Child A sustained a small cut on her finger that bled. Staff did not notify the parents of the minor injury. The parents of Child A contacted management regarding the injury.
