This list shall be dated and kept confidential in each room or area where children are present.
In the mornings, the school-age children eat breakfast in the preschool room. There were two school-age children present with sensitivities, but their names were not on the allergy list in the preschool room.

There were two allergy, sensitivity, dietary restriction lists posted in the Pre-K classroom. One list was dated and the other list was not dated. On the dated list (3/14/24) child A had a peanut, tree nut allergy . On the undated list there was a peanut, tree nut and egg allergy listed for the same child. The allergy list maintained in the two year old room was not dated.

8VAC20-780-270-A · Based on observation, not all areas of the center were maintained in clean, safe, operable condition. Evidence: 1. In the three's classroom, there was a wax melt plugged in with hot melted wax inside on the counter accessible to children in care. In the corner of the classroom, above the mini fridge, there were visible cobwebs and dust. The blue rug has rolled up/turned up corners that create a tripping hazard. 2. In the PreK classroom, there is a 16 cube cubby/bookshelf system that is unstable and presents a crush hazard. 3. In the van used for transportation, there were visible food debris, crumbs and dirt all over the floor in between the two back seats. There were two expired booster seats. 4. On the two's playground, there were16 screws on the fencing that extended from 6 threads to 18 threads long that created scratching and impalement hazards. There were leaves and debris observed underneath the Step2 slide/climber that created a potential for animal/reptiles. There was an unstable umbrella on a stand that created a crush hazard. 5. On the PreK/School Age playground, the helicopter climber had two loose bolts that created an entanglement hazard. There were 8 screws on the fencing that extended from 9 to 18 threads long that created a scratching or entanglement hazard. There was a mesh bag that was tangled and tied to the fencing that created and entanglement hazard. On the stairs of the large climber, the stripping material is worn and broken in areas creating a tripping and scratch hazard.
