On 9/04/2025, child A (Age 2 years) was receiving a diaper change by Staff A on a mobile changing table in the bathroom of the two year old classroom. Child A was in the seated position on the changing table and turned to retrieve a diaper wipe. While turning, the child pushed their hands on the wall which caused the changing table to move. The wheels on the changing table were not in the locked position. Child A subsequently fell between the changing table and the wall, hitting their forehead on the floor. Child A sustained a bump to the right side of their forehead. Child A was later taken to the emergency room for evaluation.

Evidence: The facilities safety manual states that immediate application of ice in a plastic bag or a frozen sponge in a plastic bag will be applied to any bumps or bruises sustained at the facility. In the older infant room on 08/22/2024 at 9:20 am, child A (DOB: 09/27/2023) sustained two bumps to the back of their head. It was reported that staff briefly held a cold water bottle to the child's head and management was notified about the injury.
