Evidence: Child A has an allergy to eggs and diary, child B has an allergy to dairy, eggs, fish and grains, child C has an allergy to milk, child D has an allergy to milk and child E has an allergy to gluten. The center was unable to provide an allergy care plan for these five children.

1. Three cleaning solution products were observed attached to a trash can that was located in the hallway near the exit closest to the two year old classroom. 2. A bottle of adhesive remover was observed on the bottom of the food-cart that was located in the hallway outside of the kitchen.

8VAC20-780-340-A · When staff are supervising children, they shall always ensure their, care, protection, and guidance.

On 9/04/2025, child A (Age 2 years) was receiving a diaper change by Staff A on a mobile changing table in the bathroom of the two year old classroom. Child A was in the seated position on the changing table and turned to retrieve a diaper wipe. While turning, the child pushed their hands on the wall which caused the changing table to move. The wheels on the changing table were not in the locked position. Child A subsequently fell between the changing table and the wall, hitting their forehead on the floor. Child A sustained a bump to the right side of their forehead. Child A was later taken to the emergency room for evaluation.

A portable heater observed in the Pre-K Classroom (Ages 4-5 years) was plugged in by an extension cord that was located near the entry way of the classroom and stretching down the hallway towards the exit, posing a tripping hazard. 2. The portable heater in the Pre-K Classroom (Ages 4-5 years) was located near the entryway of the classroom and accessible to children. The heater was hot to the touch.

8VAC20-780-40-E · Based on interviews and documentation review it was determined that the facility did not always maintain compliance with their own policies and procedures. Evidence: The facilities safety manual states that immediate application of ice in a plastic bag or a frozen sponge in a plastic bag will be applied to any bumps or bruises sustained at the facility. In the older infant room on 08/22/2024 at 9:20 am, child A (DOB: 09/27/2023) sustained two bumps to the back of their head. It was reported that staff briefly held a cold water bottle to the child's head and management was notified about the injury.
