8VAC20-780-80-B-2 · The center shall inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.

Evidence: At the time of the inspection the Licensing Inspector (LI) became aware of an injury requiring outside medical treatment that had occurred at the center on 5/14/25. The Site Director reported that the injury had not been reported and they were unaware of the requirement. LI requested that they report the injury during the inspection.

8VAC20-780-160-A · Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with the children.

The screening for Staff #1 was completed four days after their employment date.

8VAC20-780-400-B · Based on interviews and review of video footage it was determined that behavioral guidance was not always constructive in nature, and age and stage appropriate. Evidence: On 06/01/2022 at 8:42 am, child A (Age: 10 years) is seen stepping onto a chair attached to a table in the cafeteria. The reaction of staff A to this behavior was to strike the child on the right arm and redirected the child to the stage by moving the child by the back of their neck. Staff A briefly speaks to child A regarding the incident and instructs the child to sit down at the table alone.
