One criminal history check was obtained on 10/07/2020, and there was no documentation of a criminal history check being completed for the other state. 3. An out of state central registry finding is required to be sent off within 30 days of employment. The out of state central registry findings had not been completed for either state. The requests should have been made by 10/08/2021.

On 09/23/2021 the licensing inspector reviewed a medical authorization from for Child #1. The authorization form was signed by the parent on 06/28/2021. The authorization form expired on 07/09/2021. 3. The medication should have been disposed of by 07/23/2021.
