Evidence: 1. Staff #2, date of employment 09/08/2021, indicated on the sworn disclosure statement dated 08/13/2021 that they had resided in two states outside of Virginia within the past five years. 2. A criminal history check was required before the date of employment. One criminal history check was obtained on 10/07/2020, and there was no documentation of a criminal history check being completed for the other state. 3. An out of state central registry finding is required to be sent off within 30 days of employment. The out of state central registry findings had not been completed for either state. The requests should have been made by 10/08/2021.

Evidence: 1. The center's medication policy states "that medications that are not picked up by the parent within 14 days after authorization expires will be disposed of either by dissolving down the sink or flushing down the toilet." 2. On 09/23/2021 the licensing inspector reviewed a medical authorization from for Child #1. The authorization form was signed by the parent on 06/28/2021. The authorization form expired on 07/09/2021. 3. The medication should have been disposed of by 07/23/2021.
