The center's safe sleep policy indicates that staff members are required to complete online training regarding preventing shaken baby syndrome and safe sleep practices and staff members are required to follow the guidance that is given in these trainings at all times. Interview with staff 7 confirmed that the policy does not allow infants sleeping on Boppy pillows or having blankets. 2. In the Kind Koalas infant room, a three month old infant was observed sleeping on the floor on a Bobby pillow with a security blanket over the infants face.

Evidence: 1. Three children, age three, left through a gap in the fence in the playground. The staff saw the children leave and immediately went to get the children. The fence is tall and staff could not see the children through the fence. 2. Staff 1 stated she saw the children squeeze through the fence and immediately asked for help from coworkers to go get the children. They did not look over the fence and watch the children until someone was able to get to the them.
