8VAC20-780-80-A · For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure as it occurs.
On January 21, 2026, there were 15 children present in the Preschool classroom, but the attendance sheet showed arrival of 16 children. One child had left, and the departure was not recorded. On February 3, 2026, four children were present in the School Age classroom, and the attendance sheet showed only three children had arrived. There were 14 children present in the Preschool classroom, but the attendance sheet showed arrival 15 children. One child had left, and the departure was not recorded.

When there is evidence that the safety of children may be jeopardized by contact with a staff member or volunteer because of the physical health or mental health of such staff member or volunteer, the licensee shall, at a minimum, prohibit the employee or volunteer from engaging in contact with the children or participation in the food service program until a physician or a clinical psychologist skilled in the diagnosis and treatment of mental illness confirms that any risk has been eliminated or can be reduced to an acceptable level by reasonable accommodations. At least three staff were required or permitted to work directly with children after being diagnosed with a communicable disease. For at least two weeks, multiple classrooms throughout the center had at least 19 cases of children and staff with conjunctivitis, influenza, Group A Streptococcal (GAS) Diseases (impetigo and strep throat).

On June 17, 2026, a child was sent home from the center after being discovered that the child had lice. Parents of exposed children in the classroom had not been notified as of the date of this inspection.

On May 12, 2026, the attendance record for the Infant B classroom had three children marked as present but they were not in the group. The record for the Infant C classroom had two children marked present but they were not with the group. The record for the Preschool 1 classroom had one child marked present that was not with the group. Some of these children had left the center and others were present in different classrooms. On June 12, 2026, the attendance record for the school age classroom had one child marked present but they were not with the group. The attendance record for June 9-11, 2026 for the school age classroom was blank.

The Code of Virginia requires persons employed in a child day program that have reason to suspect that a child is abused or neglected shall report the matter immediately to the county or city wherein the child resides or wherein the abuse or neglect is believed to have occurred or to the Department's toll-free child abuse and neglect hotline. In early February 2026 five staff saw marks on Child #1 and suspected an incident of child abuse but did not report to the Department of Social Services.

In the School Age Classroom, the wood on the cabinet above the counter was broken and had rough edges, exposing the hinge from the outside which had sharp edges both create a scratching hazard. There is a bookcase with a broken area on the top exposing the wood material inside that has rough edges and creates a scratching hazard.

In the City Play Place room, there was a sprinkler head in the ceiling of a playhouse that was missing the cover which exposed sharp edges within reach of the children. In the Preschool Two room, there was visible dirt on the wall around the light switch in the bathroom. In the Two?s room, within reach of the children, there was a piece of the shelf missing exposing the particle board and a wood dowel.

8VAC20-780-400-B · Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.
The following occurred on July 30, 2025: Staff #1 and Staff #2 required thirteen two-year-old children to stay seated at their tables for approximately ten minutes while staff cleaned up after an activity. Staff #1 forced children onto chairs on seven occasions, got within an inch of a child's face to correct behavior on ten occasions, and pulled children by the top of their arms on three occasions. During one of those occasions, Staff #1 held the top of a child's arm for approximately 30 seconds while the child tried to get away and then while holding the top of the child's arm pulled the child around the room and forced the child onto a chair. Staff #2 put their face within an inch of a child's face in attempt to correct behavior on four occasions and forced a child onto a chair. Children had to stand in line for approximately two minutes before leaving the room, during which one child grabbed another. Staff #2 grabbed both of the child's hands and held them together above the child's head for at least one minute. Staff #2 pulled a child by the ankle to prevent the child from leaving the group.
