Evidence: The outdoor playground equipment multiple areas are peeling. The building paint is peeling on multiple areas. Pictures were taken of the peeling paint. Confirmed with staff. Indoor there was a broken table in area near the restrooms and kitchen entry. Confirmed with staff.

Evidence: On November 15, 2023, at approximately 7:28am, 11 children were left alone in the classroom without sight supervision of staff for about 30 seconds. During that time, Staff #1 was assisting a child in the restroom and Staff #2 and Staff #3 were in the office.
