Evidence: 1. On the Pavilion playground area there is a compost area that has three steel T-post fence posts that are sticking out of the compost pile that create an impalement hazard. 2. On the Pavilion playground, there are five steel T-post fence posts leaning against a tree with sharp protrusions at the bottom of the post. One of the posts was visibly rusted. The t-posts create a variety of hazards for falling/head/body, cutting/skin snagging, and impalement. 3. On the Pavilion playground, a slackline obstacle course type climber is installed between trees by way of ratchet strap/tie down straps. There is no verification of the safety of installation for use of preschool or school age children. 4. In the garden area, there were four steel rebar posts used to secure plastic planters on three concrete edges and one rebar was completely exposed. The rebar creates impalement and snag/cutting hazards.
