An Ouch Report dated 10/08/2025 for C1 documented the child tripping and falling over a toy and hitting his mouth on a shelf and bit lip. During a complaint investigation initiated 04/06/2026 it was determined that the Ouch Report was inaccurate and S1 engaged in a ?physical struggle over a security blanket? with C1 and causing C1 to fall forward into a classroom shelf and sustain a chipped tooth.
