Evidence: On 8/20/24, the rubber mulch under and around the playground equipment was observed to be worn down to the weed block matting with no mulch cushioning in several areas.

    22.1-289.031-B-2 · Based on review of documentation and interview, the program did not implement their procedures for appropriate supervision of all children in care. Evidence: On 11/28/23, from approximately 2:55pm until 3:00pm, Child A, a two-year-old, was left unsupervised in a bathroom. The program?s bathroom supervision policy states, ?As a safety precaution, students are to be accompanied to the bathroom or watched from the door. No child should be sent to the bathroom alone?One teacher must always remain at the entrance of the bathroom, when a child is present.? The Director reported that staff are also trained to do a headcount of the children before leaving a location and upon arriving at another location. At the time of the incident, there were 10 children present with Staff #1 and Staff #2. Staff #2 took the children?s cots to storage down the hall and around the corner, and Staff #1 sent Child A to the restroom in the hall approximately five feet away from the classroom doorway, and then left the doorway to sit on the carpet with the other children. Then, at approximately 2:55pm, Staff #1 and Staff #2 took nine children down the hall, through a door, and down another hall to the program?s indoor gym space. A headcount was not conducted in the classroom or upon arriving at the gym. At approximately 3:00pm Child A was discovered crying in the bathroom by Staff #3.
