Evidence: At approximately 3:00pm on 2/14/22, during afternoon snack/class Valentine's day party, Child A was served a "Nutri-Grain- like" bar by Staff #1. The allergy care plan on record at the center for Child A contains documentation of food allergies to wheat, eggs, peanut, dairy, and soy. Approximately 45 minutes later Child A developed a rash and threw up. Staff #1 called the director who followed Child A's allergy care plan and administered epinephrine and called 911. Child A was transported by ambulance with their parent to the hospital.
