Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, rusting of any equipment, chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment. In the preschool room, the sheets used to cover the cots during naptime were stained and the oversized pillows used on the floor for sitting had multiple stains. In the Infant Classroom the following items were observed; the sheets on the cribs were stained; the covering on the infant bouncers were stained; the paint was chipped and peeling along the wall near the play kitchen area; the rug being used by infants during reading time had multiple stains and loose particles on the rug. In all three classrooms, the floors had an accumulation of dirt and stains along the wall borders and corners of the rooms.
