When staff are supervising children, they shall always ensure their care and protection. 1.Child #1 (4 years) fell on his hand on 11/17/25 at approximately 8:19am and came crying to Staff #1 to tell them he was hurt. Staff #1 did not see Child #1 fall and asked Child #1 what had happened and if he needed ice, which Child #1 declined. 2. Staff #1 provided no further assessment to determine if additional care was needed and none was provided to Child #1. When the parent arrived at 4:29pm to pick up their child the parent saw that Child #1?s hand was noticeably swollen and stated that it hurt him to put a coat on. 3. Child #1 was taken by the parent for outside medical attention/treatment and Child #1 was diagnosed with a fracture of the left hand.
