Evidence: 1. In the toddler room #1, there were two children observed eating cracker and walking. 2. Toddler staff confirmed the two children were walking and eating in the classroom.

8VAC20-780-270-A · Based on observation and interview, it was determined that the center did not ensure that areas and equipment of the center be maintained in a safe and operable condition.

Evidence: 1. In the infant room, 5 cribs had crib sheets that were loose and not fitted to the crib sized mattress. The 5 cribs are assigned to children in care that was present at the time of the inspection. The loose fitting oversized sheet poses as a suffocation hazard. 2. Staff #1 and Staff #2 confirmed that the crib sheets for the 5 cribs had sheets that were too loose and did not fit well to the crib sized mattress.

Evidence: Staff 4 stated that an allergy notice is kept in the classroom where a child with an allergy is in care. The list is not dated and is not kept in each room where children are preset.

8VAC20-780-550-O · Based upon review of documentation and interviews, the facility has not ensured that a parent is notified by the end of the day of any know injuries to their child.

Evidence: 1. The written injury record log provided by the facility indicated an injury to child 1 occurred on 11/09/2021 at 2:30 PM. The same injury record log indicated that the parent was notified on11/09/2021 at 8:45 AM. 2. Interviews revealed that the parent of child 1 spoke to the facility staff in the morning on 11/10/2021 to inquire as to the child's injury. 3. Staff interview revealed that staff documented child 1's injury in a log on 11/9/2021, however staff did not notify the parent at pick-up the same day as the injury.
