After picking child #1 up from the center, the parents contacted the center following the discovery of a bruise on the right side of their child's forehead. Staff confirmed the child had fell and hit her head and that she had failed to provide them with an injury report.

Evidence: On 02/09/2024 for approximately 20 minutes between 7:55am and 8:15am, Staff #1 had a group of 20 children ranging in age of 3 years-5 years with no other staff present. a. The ratio for the youngest child who was 3 years-old is 1:10. b. Attendance sheets confirmed staff #1 had 20 children my 7:55a. c. Staff and the Director confirmed that the center was out of ratio from 7:55am-8:15am.
