Evidence; 1 - Upon entering the center at approximately 11:30am Child #1, a 14 month old, was observed sitting in wheelchair in the infant care room a lone. There were not any adults in the room. There were not any additional children in the room. a. The type of wheelchair Child #1 was sitting in required another person to maneuver the chair around. The child could not self propel the chair. 2 - Additionally, there was a large changing table blocking the doorway from the infant room leading into the two year old room. This doorway was the only entry to and from the infant room from inside the building.
