The center must request repeat background checks every five years. A repeat criminal history background check and central registry search for Staff #5 were not obtained until two months after the background checks had expired.

'Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards (six inches). On 8/18/25, when a serious injury of a child occurred, there was not the minimum required amount of resilient surfacing on one of the center's outdoor play areas.

The center did not follow their supervision policy and procedures, which states that "staff members must be able to see and hear the children in their care." On 5/16/25, a child left their group who was outside, for less than two minutes. The child entered the facility with another group. At the time of this occurrence, Staff #1, who was in charge of caring for the child, was not able to see and hear the child.

In the main outdoor play area, the resilient surfacing was measured in two areas: by the rock wall was approximately four inches and under the tri-rung bars was approximately one inch. The measurements did not meet the required minimum safety standard of six inches. Children were observed playing in this play area at the time of the inspection.
