The center director confirmed that the tuberculosis screenings for staff 1 and staff 2 were not completed within the required timeframes.

1. Staff #1 confirmed that she did not complete the name to face attendance when back in the classroom. Staff #1 stated ?I screwed up? and was accountable for what happened with leaving a 22 month old child alone on the playground. 2. Staff #2 stated she had no communication with Staff #1 and was unaware of the number of children that went inside the classroom and how many remained outside with her. 3. Staff #3 stated that she found Child #1 outside on the toddler playground alone without staff supervision when she took her preschool classroom outside. Staff #3 stated that it was at least 10 minutes that the toddler child was left unattended. 4. Staff #3 took Child #1 back to the classroom and the teachers and children were on the carpet doing story time. Staff #1 and Staff #2 had no knowledge a toddler child was missing from their classroom.
