Evidence: 1.During a review of the preschool bathroom on 5/13/2024 the inspector observed dirt, grime, and a dark yellow substance in the following areas. a.There was a dried dark yellow substance under and around the hinges of the toilet seat and lid. b.There was a dried dark yellow substance on the side and back of the porcelain base of the toilet. c.There was a dried dark yellow substance on the floor tile at the back of the toilet. d.There was brown dirt and grime on the left side of the porcelain toilet base. e.There was dirt and grime on the floor on the right side, directly beside and behind the toilet tank. 2.The bolt caps to at the base of the toilet were missing. The bolts were sticking up and were accessible to children.

The hole also has exposed broken and crumbling drywall. 2. The center director confirmed that there is a hole in the wall of the preschool bathroom with exposed pipes and crumbling drywall.
