The center did not have a carbon monoxide detector installed in the building.

8VAC20-780-160-C · Based on record review and interview, it was determined that the center did not ensure that at least every two years from the date of the initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening.

Evidence: 1) Staff #1 has a tuberculosis screening with a last known date of 12/5/2019, and Staff #2 has the last known date of 11/4/2019, which is outside of the required timeframe of two years and that neither Staff #1 nor Staff #2 had documents to indicate that they had completed the required screening. 2) Administration acknowledged that a new TB screening was required two years from the date of the last screening.
