Staff 1 stated that on 9/4/2024, a written record of attendance was not kept as children were boarding the school bus and as the children were departing at their bus stop.

Evidence: According to staff interviewed, child 1 receives behavior health services from a therapist while at the licensed facility. Staff confirmed that on occasion the therapist works alone with child 1 without the supervision of facility staff.

Evidence: The door to the sprinkler/electrical room was unlocked. Inside the room are numerous electrical and other wires and machinery within reach of children. There was also a fire extinguisher sitting on the floor of the closet.
