Some children at the tables were still eating. The tables had spills from lunch on them and were not sanitized before the books were being placed on the tables.

Evidence: 1. Staff interviews and review of attendance records revealed that on April 24, 2023 infants arrived into care earlier than the usual times resulting in more than four infants with one caregiver for approximately fifteen minutes until a second staff person came on duty in the classroom. 2. Staff interviews revealed that occasionally there can be one or two infants over the required 4 children per caregiver until additional coverage can be arranged.

Evidence: 1. Both staff 3 and staff 4 stated during interview that if a child in their mobile infant classroom is upset or crying and it is not during the rest period of 11:30 AM and 1:30 PM, the infant is placed on their stomach on the large shredded foam pillows and staff pat their backs. The pillows are soft, allowing for an infant?s head to sink into the pillow, a potential suffocation hazard. Staff stated the pillows are not sanitized between use, nor are they individually assigned with pillow covers, thus presenting a possible cross contamination issue. 2. Staff 3 stated and staff 4 witnessed that on 5/8/2023, staff 1 placed child 1, 13 months old, on the child?s stomach on the large shredded foam pillow so that staff 3 could pat the child?s back.
