Based on observation and interviews, it was determined the facility did not ensure all areas and equipment of the center shall be maintained in a safe and operable condition. Evidence: 1.On the preschool playground there was a hole, approximately 8 to 10 inches deep, in the grass beside the sandbox. This hole created a tripping hazard. 2.In the School Age Club room there was a hole in the wall that contained exposed wires and a screw. The hole was low on the wall in an area accessible to children. 3.There was a split wooden board in the Construction Lab. 4.In the older infant play area, there was chipped paint and exposed dry wall in an accessible area to children. 5.In the Older Two?s classroom, the corner of an art easel was broken. 6.Staff #7 confirmed these areas were in need of repair.

Evidence: 1.In the Rec Room a baseboard was peeled way approximately 14? from the wall and the veneer on the seat of a chair was cracked and peeling causing a scratch and splinter hazard. 2.In the Nook, the outside edge of the sisal carpet was lifted and curled up, causing a trip hazard. 3.Staff #7 (Assistant Director) confirmed the carpet in the Nook and the baseboard and chair in the Rec Room needed repairs.

Evidence: 1. In the Blossom?s classroom two tubes of diaper ointment were stored in a drawer that was accessible to children. 2. The licensing inspector observed a two year old child open the drawer that the diaper ointment was stored in.

Staff #11 (Program Director) confirmed that the medication had been present in the center for more than 14 days since the expiration of the written authorization.
