On 01/24/2025 Staff A administered an undetermined amount of medication to 3-year-old child #2 that was provided to the center and intended for 5 year-old child #1.The center reported the child's mother verbally told staff A the bottle contained a prescription medication. Staff A accepted this medication in an unlabeled amber bottle, not in its original packaging and without the parent's signature on the medication authorization form. Staff A did not review the medication authorization form prior to giving child #2 the medication. The center was unable to provide the dosage amount that was given to the child.

Evidence:. 1. On 04/18/2024, Staff #1 pinched 3 year-old child #1's ear causing it to look visibly red. On the same day Staff #1 also pushed Child #2's face down resulting in the child falling back onto his cot and crying. Staff #1 proceeded to throw a blanket at the child and walk away for "not listening." a. Staff reported hearing staff #1 yell "lay down" and then witnessed Staff #1 push child #2 in the face "so hard that the child fell back on his cot." b. Multiple staff witnessed child #2 crying. Staff reported when they asked the child why he was crying he pointed to and verbally said because of Staff #1. c. Staff reported child #1 told her that staff #1 pinched his ear. Staff reported child #1's ear was visibly red after the incident. The director confirmed the child's ear was red.
