Evidence: 1. On 6/30/21, child #1 was picked up from the facility and placed in his car seat. According to the parent of child #1, child #1's face was flushed and he was crying. The parent then noticed a red blister on the child's right hand. Upon noticing the blister the parent of child #1 then went back to the facility to inquire what had happened. 2. The parent of child #1 went back into the facility and spoke to staff #1 (Program Director) about the blister, but staff #1 was unaware of any injury to child #1 that would have caused the blister. 3. Staff #2 and staff #3 who cared for child #1 during the day both also said they were unaware of how child #1 received the blister to his hand. 4. Staff #1 (Program Director) confirmed that the facility did not ensure the care and protection of child #1 due to the injury (blister) on the the hand of child #1 that occurred while child #1 was in care at the facility on 6/30/21.
