Areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition. In the 2 year old classroom the baseboard lining is peeling off. In the toddler and 2 year old playground area, there is a bike with a broken frame of the tire and a plastic playhouse with a cracked ledge. These areas pose pinching and injury hazards to the children in care.

Per the center?s supervision policy, ?Children shall be supervised at all times.? Per the center?s transportation policy, ?The van driver will ensure that all children are off the van.? After Staff #1 parked the van at the center, Child #1 was left on the transportation vehicle alone for approximately 20 minutes without adult supervision.

Evidence: 1. Staff #3 (date of hire 2/01/2024) disclosed and signed on their Sworn Statement dated 1/25/2024, that they lived outside of Virginia the past five years. There was no out-of-state background check completed for Staff #3. 2. Staff #5 (date of hire 8/22/2023) disclosed and signed on their Sworn Statement dated 08/22/2023, that they lived outside of Virginia the past five years. There was no out-of-state background check completed for Staff #5. 3. Staff #1, Director, confirmed that both Staff #3 and Staff #5 did not have the required out-of-state background checks completed.

8VAC20-780-270-A · Based on observation and interview, it was determined that the center did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition.

Evidence: 1. In the infant room, the edge of the carpet was tearing off the floor with shreds of string hanging off the carpet which poses as a injury and tripping hazard to the infants in care. 2. In the 2 year old classroom, there were pieces of the baseboards in the play center of the classroom that were peeling off the wall and some parts of the baseboard missing. This poses as an injury and pinching hazard to the children in care.

Evidence: 1 - There was no information regarding allergies and restrictions for child 1 available in the two year old classroom where the child was in care. 2 - Staff 2 stated that she was unaware of the allergy needs of child 1. 3 - The emergency medication for child 1 was located in a classroom at the opposite end of the building to the classroom where child 1 was currently in care.
