Based on review of children?s records and interviews, it was determined the center did not ensure that there was a written care plan for each child with a diagnosed food allergy which included instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: 1.The record for child #4 did not contain a written care plan with instructions from a physician. a.Child #4 has a diagnosed food allergy. b.The Director confirmed instructions from a physician were not in the record.

Evidence: 1. In the four-year-old classroom there was a broken dress up play unit. The unit was missing screws at the base. The board at the base was not secure and the unit was unstable when touched. 2. There was a metal pipe approximately an inch in diameter protruding out about 4? from a brick wall in the fenced playground area. The location and height (approximately 2 ? feet from the ground) of the pipe posed an impalement hazard. There was a child size table next to the pipe. 3.The Director confirmed the equipment above was broken or protruding.
