Neither pacifier were individually assigned showing which one belonged to which child.

1. In the school-age classroom there was peeling paint on the walls, in reach of children. 2. On the playground there were 2 tricycles that each had a foot pedal missing. 3. On the playground, the plastic play structure had a cracked area where a toy steering wheel once was. The cracked area was jagged to-the-touch and posed a pinch hazard.
