On the twos playground there were 2 plastic wagons that had broken pieces with jagged and sharp edges. 4. In the school -age room, there were four 1/4 inch holes in a wall.

    22VAC40-185-330-B · Based on a record review and observation the center did not ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99.

    Evidence: 1. The toddler playground had a unitary surface made of synthetic turf underneath the play structure, which also had pea gravel mixed into the turf. Pea gravel was also observed in the fall zones of the play structure. 2. When playground surfacing is mixed, the Impact Attenuation, which is the measure of a playground surface's shock absorbing properties, can no longer be guaranteed according to the ASTM F1292's specification for Impact Attenuation.

    Video footage confirmed, on January 27th, around or about 10:15am staff in the 2 year-old room did not use appropriate behavioral guidance. 1. Staff B picked-up 2 year-old child #3 from under a sensory table in the classroom. While holding the child close to the face, it was reported, Staff B used a firm and harsh tone to discipline the child. 2. Staff A grabbed 2 year-old child #1 by the wrist to move them. 3. Staff A grabbed 2 year-old child #2 by the front of the shirt to move them from where they were seated. 2. Staff A removed her mask, pointed her finger and leaned close to child #3's face; It was reported Staff A verbally reprimanded the child at the same time.
