A table used by school age children is broken and cracked. 2. In the infant room: a. There is a broken cabinet door in reach of children in care; b. The half door has peeling paint and wood in reach of children in care; c. The plastic mat used as flooring is peeling shedding plastic.

Areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition. Evidence: 1. In the school age classroom, there is an approximate 6 inch hole on the ledge of the window sill with small wood pieces fraying from it. 2. In the Pre-K classroom there is a broken plastic basket that poses as a pinching hazard. 3. In the 2-year-old and Pre-K classroom, there are bookshelves with warped and damaged edges in reach of children in care that can be a snagging and/or injury hazard. 4. In the 2-year-old classroom, there are two boxes of disposable gloves on the changing table area that are in reach of the children in care that poses as a suffocation hazard.

8VAC20-780-270-A · Based on record review and interview it was determined the center did not ensure areas and equipment of the center be maintained in a safe and operable condition.

Evidence: 1.In the three year old classroom, the play kitchen set had two separate areas of cracked plastic. 2.In the two?s classroom the television placed on top of a shelf was unsecured, which poses as a tipping hazard. The television was in reach of the children. 3.Staff #1, the center director, confirmed that the kitchen set equipment had cracks and the television was unsecured on the shelf.

8VAC20-780-60-A · Based on record review and interview it was determined that the center did not ensure to maintain and keep at the center a separate record for each child enrolled which shall contain work phone numbers of each parents who has custody.

1. Child #1, Child #2, and Child #3?s records observed did not have a parent?s work phone number. 2. Staff #1 and Staff #2 confirmed that Child #1, Child #2, and Child #3?s records did not have a parent?s work phone number.

8VAC20-780-350-B-2 · Based on observation and interview, it was determined that the center did not ensure that children of the ages from 16 months up to 24 months have the ratio in their classroom of 1 staff member to 5 children.

Evidence: 1. In the toddler classroom, ages 16 months to 24 months, the classroom ratio at 7:55am was 1 staff member to 6 children which is outside of the ratio requirements. 2. Staff #2 confirmed that there were 6 children in their care which was out of ratio and over by one child.

8VAC20-780-570-A · Based on observation and interview, it was determined that the center did not ensure that when a child is placed in an infant seat or high chair, the protective belt shall be fastened securely.

Evidence: 1. In the infant room, there were seven children eating lunch at the feeding table without protective belts fastened. 2. Three of the seven infants were observed climbing out of their seats during lunch. 3. Staff #1 confirmed that the seven children at the feeding table did not have protective belts securely fastened.
