8VAC20-780-270-A · Based on observation and interview, it was determined that the center did not ensure that areas and equipment of the center shall be maintained in a safe and operable condition.

Evidence: 1. The large fish tank stand located in the infant classroom has two cabinet doors with no protective safety lock that are in reach of children. Children at the center can reach and open it which poses as an injury hazard. 2. Infant staff members confirmed that there was no safety lock on the two cabinet doors for the fish tank stand and that children can reach and open the cabinet doors. 3. The electrical cords from the swings to the electrical outlet were exposed and loose for children to grab when crawling on the ground. This poses as a choking, tripping, and injury hazard to the children in case. 4. The infant staff members confirmed that the electrical cords to the swings were loose and were accessible to the children in care.
