    Repeat Violation Based on observation, the center did not maintain a written record of daily attendance that documented arrival of children as it occurred. Evidence: 1. The Two's room did not have attendance completed at 9:00AM, with 12 children present. 2. The 3's room and Pre-K rooms combined on the playground did not have attendance documentation with the group at approximately 8:50 AM. Two children arrived to the group at that time. Attendance documentation was in the classroom, and not completed.

    8VAC20-780-340-A · Based on interviews the center failed to always ensure care, protection and guidance of one child.

    Evidence: 1. On 7/19/23 Child #1 (Age 3 years) was alone and unsupervised outside of the center in the parking lot and on a neighborhood street that was approximately 72 yards from the fence surrounding the playground. 2. The child?s classroom was on the playground behind the building, having water day activities. 3. Multiple staff stated they had removed the child from the fence when he tried to climb over the fence. 4. On 7/19/23 a community member witnessed the child running up the parking lot towards her car. She parked and followed the child while calling 911. 5. Local police arrived to pick up the child and community member and returned them both to the facility at approximately 12:25pm. 6. Staff were unable to explain how the child got out of the center.

    Systemic Deficiency Based on interviews and observation, children under 10 years of age were not always within actual sight and sound supervision of staff.. Evidence: 1. On June 18, 2024 at approximately 9:30 AM, Child 3 (20 months old) and Child 4 (17 months old) exited the Toddler room, to the outdoors into an unfenced area, through an open exterior door. Staff had left the door open while taking out trash. The dumpster is located in the parking lot of the center, approximately 75 feet away from the classroom. Staff stated the children were out of sight and sound supervision for approximately one minute. 2. On July 1, 2024, during the inspection, Child #2 (4 years old) was observed entering the vacant Pre-K (4's) classroom, alone and without supervision, from the vacant Preschool classroom. The child retrieved a jacket and exited the classroom through the back door to the playground area. 3. On July 1, 2024, during the inspection, Adult C (health professional) was observed working alone with Child 1(4 years old) by the inspector upon entering the Pre-K classroom. Adult C is not a staff member and there is no documentation authorizing the individual to pick up the child.
