The record for Child A, B, (enrolled for over a year) and Child E (enrolled for 7 months) were missing the addresses of the emergency contacts. The record for Child D, enrolled for 7 months had the parents listed the emergency contacts. The record of Child E did not contain work phone number and place of employment of one of the parents.
