22.1-289.031-A-2 · Based on record review, observation of the physical plant, and staff interview, the center did not have documentation from the local fire marshal that the physical facilities of the child day center have been inspected and determined to be in compliance with applicable laws and regulations within regard to the Statewide Fire Prevention Code or documentation of the most recent health inspection.

Evidence: There was no documentation on file of the most recent fire inspection conducted by the fire marshal. There was a notice posted on the door to the center from the Prince William County Office of the Fire Marshal stating "This structure has been placed on Fire Watch by order of the Fire Marshal." The notice further stated the reason for Fire Watch is "Fire and sprinkler out of service". The building was placed on Fire Watch on 1/4/23. There was also no documentation on file of the most recent health inspection conducted by the local health department.
