8VAC20-780-340-A · Based on interviews, a review of record keeping and observation, it was determined that the center failed to ensure the care and protection of a child in care.
Evidence: On 7/13/22 staff A told a parent to sign the medication authorization form and that they would fill in all the other information required on the medication form. The parent signed the form and left and staff A proceeded to filled in the information. Staff A wrote on the form that child A was to be given 20ml of multi-symptom cold medication every 4 hours and not the 10 ml every 4 hours as directed by the parent. The multi-symptom cold medication was given to child A ( 8 years of age) at 9 am and 1pm. The child was picked up from the summer camp at approximately 4: 45 pm by a parent, who inquired why their child looked so tired. It was then discovered that the center administered the wrong doses of medication.
