On August 2, 2024, Child#1 (age 18 months ) was sitting at a table with other children, and was one of two children strapped into a toddler chair with a booster seat attached to it. Child#1 pushed away from and tilted the table forward with his hands and Staff#1 pulled the chair the child was strapped in and pushed him to face a wall. Child#1 used his feet to push away from the wall and Staff#1 picked the chair up with the child still strapped inside and carried him to another table across the room. Child#1 rocked the chair back and forth and attempted to exit multiple times before falling over with the chair still attached to his body sustaining an injury to his lip. The booster seat, which was attached to the toddler chair, was only intended to be used with dining chairs.
