Evidence: 1. On January 22, 2024, the center did not have sufficient heat from approximately 6:30am until approximately 11:46am. A work order documented 2 hours of work to ?reset the freeze stat? on January 22, 2024; the work was completed at approximately 11:46am 2. Staff interviews confirmed that on January 22, 2024, the center was ?freezing? when they arrived, some as early as 6:30am. This was reported to management when they arrived at approximately 7:00am. 3. At approximately 7:00am, management noted the temperature gauge inside the center was registering 58 degrees in the Preschool 1 and 2 Classrooms. They turned on all the bottom heaters in all the classrooms, but it was still cold. The Preschool 1 and 2 children were then moved into the Toddler classroom. 4. Management called the contractor at approximately 7:00am to repair the heating system. The contractor arrived approximately 2 hours later. 5. At approximately 8:30-9:00am, staff put the infants in their coats and/or blankets. Staff observed that the infant's lips, face, hands and feet were turning purple/blue. 6. Staff felt the heat start pushing out at approximately 10:52am. 7. Work was completed on the heating system at approximately 11:46am.

8VAC20-780-410-1 · Based on interviews and observation of the center's video recordings, a staff member used a forbidden action of rough handling a child in care.
Evidence: 1. On 4/14/2022 at approximately 5:00pm, while Child #1 (18 months old) walked near a table in the Toddler classroom where a few of the children were sitting for snack, Staff #1 roughly grabbed Child #1?s left upper arm with her right hand, pulled out a chair at the table, picked Child #1 up off the floor by his left wrist and his right upper arm, roughly put the child in the chair and quickly slid the chair with the child up to the table. 2. While watching the center's video recordings of the incident, Staff #1 stated the following: "Made it seem I was rough.? ?It was a fast grab because of my body language, it looked rough.? ?I was wrong in the way I grabbed him." 3. Staff #2 stated "it looked a little aggressive." "I felt like, don't grab him by the arm and pick him up." 4. Staff #3 stated "be careful" to Staff #1 after she witnessed Staff #1 grab Child #1 by the arm, pull him and sit him down. Staff #3 stated it was ?inappropriate?, ?too rough? and was ?afraid [Staff #1] was going to hurt the child.? 5. Adult #1 witnessed the incident and wrote in an email to center management, Staff #1 ??forcefully grabbed him by the arm and roughly set him down in a chair.?
