8VAC20-780-340-A · Based on interview and observation, it was determined staff on March 22, 2023, did not ensure the care and protection of a child.
Evidence: 1. On March 22, interviews confirmed Staff #3 tried to guide child #1, a 23 month old, to the smaller playground by lifting the child up by one arm, twice. 2. During interviews of staff, Staff #2 stated child #1 was crying on and off while playing in the playground and then inside the class. 3. Staff #1 stated child #1 was not able to lift up the arm, and the decision was made to contact the family. 4. Child #1 was taken to the Doctor, diagnosed with a dislocated arm, and the arm was placed back in.
