Evidence: 1. On Thursday, July 11, 2024, at approximately 11:14 am, Child #1 (2 years old) was found sitting alone and unsupervised on the floor in Classroom #9. 2. Staff #2 stated they completed a head count in the hallway to ensure all seven students were present before entering the classroom at approximately 11:05 am. However, they failed to perform a second head count once inside, resulting in their unawareness of Child #1. 3. At approximately 11:12 am, Staff #2 noticed that Child #1 was not in the room and called another staff member to help in locating Child #1. 4. Staff #2 stated that they suspected Child #1 slipped out of line and walked to Classroom #9 while they were waiting in the hallway. 5. Child #1 was alone and unsupervised in Classroom #9 for approximately nine minutes.
