Evidence: On the playground, there are two portions of the poured-in-place rubber surfacing that have collapsed leaving an opening/indention that presents a potential tripping hazard. The two areas are approximately 8" by 4" in size with a depth of 1" - 2". They are located on either side of the slide.

Evidence: 1. A video was reviewed of an incident on October 4, 2022 between approximately 10:45 am ? 11:00 am. Children were observed sitting at a table. Staff #1 took a book from Child #1. Child #1 hit Staff #1 in the leg. Staff #1 responded by hitting Child #1 with her open hand on his upper right arm below his shoulder. 2. Staff #2 stated she was in the room on October 4, 2022, when the incident occurred. Staff #2 did not see the incident however she heard Staff #1 say ?Give me the books? and then there was complete silence. Staff #1 soon left the room and Child #1 began crying. Staff #2 went to the child and Child #1 stated she (Staff #1) hit him. Staff #2 reported what child stated to administration when they entered the room less than a minute after the incident.
