Evidence: Four caregiver / Assistant. / Household Member records were reviewed. The record for caregiver #3 contained the most recent results of a Criminal History Record check which were dated 1/25/2019, more than 5 years ago. The record for caregiver #3 contained the most recent results of a Search of the Central Registry check which were dated 2/12/2019, more than 5 years ago. The licensee confirmed that this was an oversight and that a new request for an updated Criminal History Record Check and an updated Central Registry search had not been completed.

8VAC20-800-170-A · Based on a review of records and interview, the licensee did not obtain from each caregiver at the time of hire and each adult household member prior to coming into contact with children a current Report of Tuberculosis Screening form published by the Virginia Department of Health or a form consistent with it documenting the absence of tuberculosis in a communicable form.

Evidence: Three caregiver records were reviewed. The record for caregiver #3, who began employment on 3/17/2023, did not contain results of a Tuberculosis Screening. The licensee confirmed that she had not received this documentation from caregiver #3 who was working with the children during the inspection.

8VAC20-800-180-A · Based on a review of records and interview, the licensee did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening, in accordance with the requirements in 22 VAC 40-111-170, every two years from the date of the first screening.

Evidence: Two caregiver records were reviewed during the inspection. The Most recent TB screening for caregiver #1 was dated 6/14/2018. The most recent TB screening for caregiver #2 was dated 2/5/20219. The licensee confirmed that she did not have updated TB screenings for caregiver #1 or caregiver #2 and had not realized that they were expired.
