Evidence: The documentation / file for caregiver #2, who was assisting in the care of children on 2/2/2020 during the inspection, did not contain results of a criminal history record information check and a sex offender registry check from the state of Illinois which caregiver #2 documented as a state of residence within the last 5 years on her Sworn Disclosure or Affirmation form. Caregiver #2 was hired 9/13/20219 but had not been working a regular schedule since January 2021. Out of state background checks were required to be obtained for current employees by 12/31/2020.
