1. There was a toy wheelbarrow in the outdoor play area with jagged edges where a handle had broken off or been removed. 2. There was a broken window on the shed located directly next to the outdoor play area. The window pane contained broken glass shards. 3. There was a protruding nail at the top of the wooden handrail leading to the outdoor play area.
