Evidence: The records for child #2 and child #3 did not contain the name and policy number of the child's medical insurance. Child #2's record had no information written pertaining to the child's medical insurance. The record for child #3 contained the insurance provider's name but no policy number. The record for child #3 did not contain the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached. Only one emergency contact person's information was documented in the record for child #3. The licensee confirmed that she did not have the aforementioned documentation for child #2 and child #3.
