    Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; and protruding nails, bolts or other components that could entangle clothing or snag skin. 1. On Playground #1 and Playground #2, there were areas of rust on the metal bars and bolts on the composite playground pieces. 2. On Playground #1, the slide was cracked and broken in the center of the slide. 3. On Playground #2, the poured-in place surfacing has multiple areas of separation as well as a four foot section that is completely separated causing a tripping hazard. 4. In Classroom D, there were areas of peeling paint/drywall, rust on the barrier in the bathroom and approximately 25 staples on the walls that could snag skin. 5. In Classroom E, there was rust on the vent in the bathroom and areas of peeling paint/drywall.
