operable condition. Evidence: 1. In the Three year old classroom, a rug was torn approximately 2 inches at the corner of the rug and was creating a tripping hazard.

Evidence: 1. Staff #1 (Date of hire 8/31/2023) had the central registry findings in the file, but it was dated 10/31/2023. The results came in after 30 days of employment due to being sent back for corrections.

8VAC20-780-40-M · Based on review and observation, the center did not maintain in a way that is accessible to all staff who work with children, a current dated written list in each room or area where children are present of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8.

Evidence: 1. The infant/Young 2's classroom had an allergy list, but it was not dated. 2. Three classrooms did not have a current dated list of all children's allergies, sensitivities and dietary restrictions.

8VAC20-780-270-A · Based on observation, areas and equipment of the center inside and outside were not maintained in a safe and operable condition.

Evidence: 1. In the 3yr-4year old classroom, the paint was chipping on the edge of the wall near the windows. 2. In the Toddler classroom, a scripto lighter and 3 adult scissors were inside an unlocked drawer within reach of children. 3. On the playground, two hula hoops were bent/broke, the green riding car had a missing part which created sharp edges, a pink bike was missing the bell and created a sharp edge, an irrigation box was not locked and could be lifted up exposing wires and a hole in the ground and a bait station was near the wall of the building.
