The record for Child #1 did not contain the complete address of both emergency contacts and the work information for both parents. The record for child #2 did not contain the work number for both parents. The record for child #3 did not contain the work number for one parent.

8VAC20-780-270-A · Areas and equipment of the center, inside and outside, shall be maintained in a safe condition. The following safety issues were observed:

1.The indoor play area material shelves had containers that held small plastic buttons, small plastic teddy bears and foam shape pieces that were small enough to be a potential choking hazard. 2.The outdoor play area had multiple wooden framed chalk boards that had spaces in between the corners of each frame that could cause a potential snagging or splintering injury. 3.The restroom had a pack of plastic garbage bags in a cabinet unlocked and accessible to children causing a potential suffocation hazard.
