Evidence: 1. The license is currently issued for the facility located at 317 N. Lewis Street, Staunton, VA 24401. No children were in care at the facility on May 23, 2022. 2. The licensee submitted an initial application for an address change on January 26, 2022, and provided the last required documentation on May 5, 2022. 3. The administrator stated the center opened at 320 N. Central Ave., Staunton, VA 24401 on May 10, 2022 despite not having the license for the new location. 4. The inspector observed children in care on May 23, 2022 at 320 N. Central Ave., Staunton, VA 24402.
