22.1-289.031-B-5 · The center is required to establish and implement procedures for ensuring that all areas of the premises accessible to children are free of obvious injury hazards, including providing and maintaining sand or other cushioning material under playground equipment.

1. On 4/17/26 the composite playground structures on the larger playground for the older children were observed to be over bare grass with no cushioning material underneath. 2. The yellow slide on one of the composite playground structures was observed to be missing two out of three bolts attaching the slide to the play structure.

On 5/8/25 there were no staff on-site with current daily health observation training.

22.1-289.031-A-3-a · Based on observation and interview, the program did not ensure that the required ratio of one staff to four children aged zero to 16-months-old was met. Evidence: On 5/15/24 at approximately 8:55am one staff was observed supervising seven children in the ?Toddlers? classroom. The youngest child in the group was 15-months-old and required a ratio of 1 staff: 4 children. The second teacher arrived at approximately 9:15am.
